If there is no other way to get a free ride to a scheduled medical
appointment, MUSTcall three days in advance to make arrangements.

   *** Your client will also need their Medicaid #, destination address,
drop off time, pick up time and Name of Doctor they are seeing.    P.S.  the
man I talked to said there was no limit for requests
