well, accessible means not only regarding how to read the material, it also means the ability to complete any and all forms necessary to continue or apply for benefits.
I would say that all forms need to be in an accessible format that can be completed on line on a Secure web page at the SSA web site with the ability to fill them out on line and then print them out, or send them securely and directly electronically to SSA.

Just a thought and maybe one which has been considered and put aside. Like many, I have received one of those forms from Social Security asking about recent work history. I don't happen to hire a reader as I do all my print activities on the PC and with the scanner. So, having scanned the document I wrote a letter, which I signed, answering all of the questions on the printed form. And as I said, I signed the document.

Fast forward one month. Today I received the document along with a second request. It appears that just answering the questions is not good enough; they must be answered with a number two pencil or black ink on their form.

So what are we asking for? Well we are asking for documentation to be provided in accessible form. However, how do we handle things when we get our docs in accessible form and still can't complete them? If anyone has come up with a solution to this issue I'd be interested in knowing about it.
