You can complete the on-line form on this page or you can download the following file to fill out and mail to the MCB office. ExpenseForm.doc The following on-line form contains three data fields which are automatically calculated as you fill in other data fields on the form. The data fields are the Total Transportation Cost, the Total Meals Cost, and the Grand Total. The calculation of these fields cannot be overridden, you must change the preceding data entries that effect the calculation instead. If any entry errors are detected after you submit the form, the form will be redisplayed retaining all of your previous entries, including error messages describing the entry errors. Simply correct the errors described and resubmit the form. This on-line form will save your contact information in a cookie file on your computer and will be used to automatically fill in the contact information the next time you access the form. The data stored in the cookie is not accessible by third parties and is safe from hackers since it only resides on your computer.
